Il existe des biomarqueurs sp\u00e9cifiques qui ont \u00e9t\u00e9 bien caract\u00e9ris\u00e9s et dont il a \u00e9t\u00e9 d\u00e9montr\u00e9 \u00e0 maintes reprises qu'ils permettent de pr\u00e9dire correctement les r\u00e9sultats cliniques pertinents pour toute une s\u00e9rie de traitements et de populations, mais dans de nombreux cas, la validit\u00e9 des biomarqueurs doit toujours \u00eatre \u00e9valu\u00e9e.<\/p>\n\n\n\n<p>Mon objectif dans cet article est de discuter de l'utilit\u00e9 des biomarqueurs dans diff\u00e9rents processus tels que le d\u00e9veloppement de m\u00e9dicaments.<\/p>\n\n\n\n<p>Un biomarqueur est une caract\u00e9ristique biologique qui est objectivement mesur\u00e9e et \u00e9valu\u00e9e comme un indicateur de processus biologiques ou pathologiques normaux, ou d'une r\u00e9ponse \u00e0 une intervention th\u00e9rapeutique ;&nbsp;<a href=\"https:\/\/www.nature.com\/subjects\/biomarkers\" target=\"_blank\" rel=\"noreferrer noopener\">exemples incluent<\/a>&nbsp;des mod\u00e8les d'expression g\u00e9n\u00e9tique, des niveaux d'une prot\u00e9ine particuli\u00e8re dans les fluides corporels, ou des changements dans l'activit\u00e9 \u00e9lectrique du cerveau.<\/p>\n\n\n\n<p>Au printemps 2015, le Conseil de direction conjoint FDA-NIH a travaill\u00e9 ensemble pour harmoniser les termes utilis\u00e9s dans la science translationnelle et le d\u00e9veloppement de produits m\u00e9dicaux en tant que besoin prioritaire, en se concentrant sur les termes relatifs aux points finaux de l'\u00e9tude et aux biomarqueurs. Leur objectif \u00e9tait d'am\u00e9liorer la communication, d'aligner les attentes et d'am\u00e9liorer la compr\u00e9hension scientifique. Les deux agences ont mis au point le&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK326791\/\" target=\"_blank\" rel=\"noreferrer noopener\">Ressource BEST (Biomarkers, EndpointS, and other Tools)<\/a>.<\/p>\n\n\n\n<p><strong>Les biomarqueurs peuvent \u00eatre inclus dans sept cat\u00e9gories :<\/strong><\/p>\n\n\n\n<p>- susceptibilit\u00e9\/marqueur biologique de risque<\/p>\n\n\n\n<p>- biomarqueurs de diagnostic<\/p>\n\n\n\n<p>- surveillance des biomarqueurs<\/p>\n\n\n\n<p>- biomarqueur pronostique<\/p>\n\n\n\n<p>- biomarqueurs pr\u00e9dictifs<\/p>\n\n\n\n<p>- biomarqueur pharmacodynamique\/de r\u00e9ponse<\/p>\n\n\n\n<p>- biomarqueurs de s\u00e9curit\u00e9<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Clinical endpoints ou surrogate endpoints ?<\/h2>\n\n\n\n<p>Comme le Dr Timbu et le Dr Tavel&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3078627\/\" target=\"_blank\" rel=\"noreferrer noopener\">a dit<\/a>:&nbsp;<em>\"Les biomarqueurs jouent un r\u00f4le critique dans l'am\u00e9lioration du processus de d\u00e9veloppement des m\u00e9dicaments ainsi que dans l'entreprise plus large de la recherche biom\u00e9dicale. Comprendre la relation entre les processus biologiques mesurables et les r\u00e9sultats cliniques est essentiel pour \u00e9largir notre arsenal de traitements pour toutes les maladies, et pour approfondir notre compr\u00e9hension de la physiologie normale et saine\".<\/em><\/p>\n\n\n\n<p>Depuis environ 40 ans, les cliniciens et les chercheurs discutent de l'importance d'utiliser des biomarqueurs comme r\u00e9sultats de substitution dans les essais \u00e0 grande \u00e9chelle de maladies majeures, comme le cancer.<\/p>\n\n\n\n<p>Pour \u00e9valuer un biomarqueur, les chercheurs et les cliniciens doivent comprendre parfaitement la physiologie normale d'un processus biologique, la physiopathologie de ce processus dans l'\u00e9tat de la maladie, et les effets d'une intervention pharmacologique sur ces processus.<\/p>\n\n\n\n<p>Malheureusement, ces processus sont difficiles \u00e0 comprendre compl\u00e8tement et les biomarqueurs en tant que param\u00e8tres de substitution doivent toujours \u00eatre r\u00e9\u00e9valu\u00e9s. En outre, les \u00e9tudes utilisant des biomarqueurs devraient toujours avoir comme mesures finales les r\u00e9sultats cliniques et la corr\u00e9lation avec le succ\u00e8s, du moins pour l'analyse r\u00e9trospective des biomarqueurs.
